NPI number lookup

Type a 10-digit NPI. See what the federal NPPES registry lists: name, individual or organization, primary specialty, the states where a license is reported, and status. Free, no account, and nothing you type is stored.

Searching by name instead? Use the NPPES NPI Registry search.

What an NPI lookup shows,
and what it does not

01

Who it is

The name on the NPI, and whether it belongs to an individual or an organization.

02

Specialty

The primary taxonomy the provider chose in NPPES.

03

States

The states where the provider reports a license. Self-reported, so a starting list.

04

Not proof

NPPES does not verify licenses. To confirm one is active, use the state board’s own lookup.

NPI basics

What is an NPI number?
A National Provider Identifier: a unique 10-digit number for health care providers in the United States, issued through CMS’s National Plan and Provider Enumeration System (NPPES).
Does an NPI expire?
No. An NPI does not expire. Providers must report changes to NPPES within 30 days.
Can I look up an NPI by name?
This tool looks up by number. To search by name, use the NPPES NPI Registry search run by CMS.
Does an NPI lookup verify a medical license?
No. The license information in NPPES is reported by the provider and is not checked against the state board. To confirm a license is active, use the board’s own license lookup.
Is anything I type stored?
No. The number is sent to the public NPPES registry and the answer is shown to you. We do not keep a record of the lookup.

In the Ledger, the same lookup offers each state license NPPES lists, so you can add each one and set its renewal date.
