CAQH re-attestation: every 120 days, step by step
CAQH now operates as DataSpring, and ProView is now called the Provider Data Portal. The sign-in is still proview.caqh.org. Checked against DataSpring’s own guides on October 19, 2026.
The short answer
Re-attest at least every 120 days, or every 180 days if your primary practice state is Illinois. DataSpring emails you 15, 10 and 5 days before the deadline. The day after it passes, your profile shows as Expired Attestation. It is free for providers. An expired license or malpractice policy has to be updated before the portal lets you attest.
How to re-attest
- Sign in at proview.caqh.org.
- Choose Review & Attest and fix anything the portal flags.
- Update expired documents first: state licenses, DEA and state controlled-substance certificates, the malpractice face sheet, board certification.
- Upload any new supporting documents, such as a signed release or W-9.
- Click Attest. Then put the next date, 120 days out (180 in Illinois), on your calendar.
If you miss it
Your status changes to Expired Attestation the day after the deadline. DataSpring sends notices 1, 14, 28 and 42 days after it expires; the last is the final notice. Health plans read your profile for credentialing and recredentialing, so an expired profile can hold up enrollment. Sign in and attest as soon as you can.
For practices and groups
At 120 days, CAQH comes due about three times a year for every clinician: more often than any license or card in the file. Track each clinician’s last attestation date, and remind them two weeks ahead.
An office manager can use DataSpring’s Practice Manager module to enter information, which the provider then reviews and imports into their own profile. DataSpring’s guides describe the provider doing the attestation.
Common questions
How often do providers have to re-attest in CAQH?
At least every 120 days. Providers whose primary practice state is Illinois re-attest every 180 days.
What happens if my CAQH attestation expires?
Your status changes to Expired Attestation the day after the deadline, and DataSpring sends notices 1, 14, 28 and 42 days later. Sign in and attest to make the profile current again.
Is CAQH free for providers?
Yes. DataSpring provides the portal at no cost to clinicians, provider groups and practices.
Can my office manager re-attest for me?
An office manager can enter and export your information through the Practice Manager module for you to import. DataSpring’s guides describe the provider attesting, so plan on doing that step yourself.
Is CAQH the same as DataSpring?
Yes. CAQH rebranded as DataSpring, powered by CAQH, in June 2026, and ProView was renamed the Provider Data Portal in 2023. Logins, profiles and the proview.caqh.org address did not change.
The Interstate Medicine Roster keeps the documents CAQH asks for (licenses, DEA, malpractice, board certification) current for every clinician, on one Due list. It does not connect to CAQH; the clinician still attests in the portal.
